Minnie Hamilton Health System uses ChartRequest to streamline your medical record exchange, simplify referral management, and ensure quality care coordination. To request copies of your medical records from Minnie Hamilton Health System, please submit an electronic request by clicking the link below. Please be aware that a signed release form is required for all records requests.

If you need more information about the price of a future service, please contact our Price Hotline at (304) 354-9244. A CPT code is strongly encouraged when you call. You can obtain the CPT code from the ordering physician.
